CKD-MBD
Also called: chronic kidney disease-mineral and bone disorder, renal osteodystrophy, kidney bone disease
What it means
CKD-MBD is a bone and mineral problem caused by chronic kidney disease. As kidneys fail, they can't properly balance calcium, phosphorus, and vitamin D, which weakens bones and affects blood vessels too.
If you have kidney disease, your bones may be silently thinning or weakening in ways a standard bone scan might not fully explain, so it needs its own monitoring and care.
CKD-MBD stands for chronic kidney disease-mineral and bone disorder, also called renal osteodystrophy. It happens when declining kidney function disrupts the body's normal handling of calcium, phosphorus, and vitamin D. Healthy kidneys activate vitamin D and clear excess phosphorus; when they can't, a chain reaction follows that affects bone strength over time.
Here's the chain: as kidney filtering (GFR) drops, phosphorus builds up in the blood and a hormone called FGF23 rises to try to compensate. Both of these suppress calcitriol, the active form of vitamin D, leading to low calcium. The body responds by releasing more parathyroid hormone (secondary hyperparathyroidism), which pulls calcium from bone, often causing high-turnover bone loss.
CKD-MBD isn't one single condition — it can show up as high-turnover bone disease, or in some dialysis patients, as low-turnover 'adynamic' bone disease, sometimes complicated by aluminum exposure. Because these look different on biopsy and need different management, doctors may order a bone biopsy, especially at more advanced kidney disease stages, to figure out which pattern is present before deciding on treatment.
Kidney disease can quietly weaken bones by disrupting calcium, phosphorus, and vitamin D balance, often undetected by standard scans.
