Glucocorticoid-induced osteoporosis
Also called: steroid-induced osteoporosis
What it means
Glucocorticoid-induced osteoporosis is bone loss caused by steroid medications or high cortisol levels, which weaken bone by slowing bone building and speeding up bone breakdown.
If you take steroid medication or carry chronic stress, understanding this helps you ask the right questions about protecting your bones over time.
Glucocorticoid-induced osteoporosis is bone loss caused by steroid medications like prednisone and dexamethasone, or by conditions that raise cortisol such as Cushing's syndrome. Cortisol attacks bone from several directions at once: it suppresses osteoblasts (the cells that build bone), ramps up osteoclast activity (the cells that break bone down), reduces how much calcium your gut absorbs, and increases how much calcium your kidneys flush out.
Bone loss from steroid medications can begin within the first few months of use, and the risk is dose-dependent, meaning higher doses and longer use carry more risk. Because this pattern is so well understood, guidelines recommend bone protection for people taking steroids for more than three months, and even high-dose inhaled steroids carry some risk.
Interestingly, the same mechanism can happen without any medication at all. Chronic stress causes your own body to produce sustained high cortisol, which affects bone in the same ways as steroid drugs. If you're prescribed a corticosteroid, any questions about dose, duration, or bone protection are best discussed with your prescriber, since they can weigh your individual situation.
