Hormonal birth control and bone
Also called: Depo-Provera bone loss, birth control pill bone density
What it means
Hormonal birth control can affect bone density depending on the type; the progestin-only injection (Depo-Provera) is linked to temporary bone loss, while pills and IUDs generally are not a major concern.
If you've used or are using hormonal birth control, knowing which types affect bone helps you and your doctor watch for risk factors over time.
Different forms of hormonal birth control affect bone in different ways. Combined estrogen-progestin pills generally have little to no negative effect on bone density in most women, and some research suggests they may even be mildly protective, since estrogen helps slow bone breakdown. The progestin-only injection (Depo-Provera) is the main exception — it suppresses the body's natural estrogen production, and this has been linked to measurable bone density loss while it's being used.
This matters most for teens and young adults still building peak bone mass, and for women using Depo-Provera for many years. The bone loss seen with this injection appears to partly reverse after stopping it, but the full long-term picture isn't completely settled. Other methods, like hormonal IUDs or implants, release very low, localized doses of progestin and aren't considered a significant concern for bone health.
If you're using or considering hormonal birth control and have other risk factors for low bone density — like a thin build, smoking, early menopause, or a family history of osteoporosis — it's worth bringing up with your doctor. They can weigh the benefits of the method against any bone-related tradeoffs and decide, together with you, whether monitoring or a different option makes sense. This isn't a decision to make without your prescriber's input.
The Depo-Provera shot is linked to temporary bone loss, while birth control pills and IUDs generally aren't a concern.
