Hypoparathyroidism
Also called: low parathyroid hormone, underactive parathyroid
What it means
Hypoparathyroidism is a condition where the parathyroid glands make too little parathyroid hormone, leading to low blood calcium and high phosphate levels.
This condition disrupts the hormone that controls calcium balance, which can affect bone structure and overall skeletal health.
Hypoparathyroidism means the parathyroid glands, four tiny glands in the neck, don't make enough parathyroid hormone (PTH). PTH normally keeps blood calcium steady by pulling calcium from bone, boosting absorption in the gut, and helping the kidneys hold onto calcium. Without enough PTH, blood calcium drops and phosphate rises, which can cause muscle cramps, tingling, fatigue, and in some cases bone and dental changes.
The most common cause is damage to or removal of the parathyroid glands during neck or thyroid surgery. Less often it's autoimmune, genetic, or linked to very low magnesium, since magnesium is needed for the glands to release PTH properly - severe magnesium deficiency can look like hypoparathyroidism even when the glands are normal.
Because PTH helps regulate how bone is built and broken down, people with hypoparathyroidism can have unusually dense or altered bone compared to those with too much PTH (hyperparathyroidism). Treatment usually involves calcium and active vitamin D, sometimes PTH-replacement therapy, managed carefully by an endocrinologist since levels need regular monitoring. Any changes to treatment should always go through your prescribing doctor.

Too little parathyroid hormone causes low blood calcium and high phosphate, which can affect your bone structure and skeletal health.
