What it means

IL-6 is a signaling protein (cytokine) made by immune cells, muscle, and other tissues. In bone, it's mostly known for ramping up osteoclast activity, which can speed up bone breakdown when levels stay chronically high.

Why it matters for your bones

Persistently high IL-6, as seen with chronic inflammation or certain illnesses, can tip your bone remodeling balance toward more breakdown than buildup.

IL-6 (interleukin-6) is a cytokine, a chemical messenger your immune system uses to communicate. It's released during infection, injury, and inflammation, but also by working muscle. In bone, IL-6 generally increases RANKL signaling, which activates osteoclasts, the cells that break down bone tissue. When IL-6 stays elevated over long periods, this can shift bone remodeling toward more resorption than formation.

This is one reason chronic inflammatory conditions, like rheumatoid arthritis or inflammatory bowel disease, are linked to bone loss over time. It's also part of why some cancers, such as multiple myeloma, cause aggressive bone destruction: cancer cells release IL-6 along with other factors that overstimulate osteoclasts while suppressing the osteoblasts that would normally rebuild bone.

IL-6 isn't purely harmful, though. It's also classified as a myokine, a substance released by contracting muscle, and research suggests it may have different effects depending on context, sometimes influencing both bone formation and resorption. This complexity is why scientists describe IL-6's role in bone as context-dependent rather than simply good or bad.
