Insufficiency fracture
Also called: fragility stress fracture
What it means
An insufficiency fracture is a break that happens in bone already weakened by low bone density, caused by normal daily activity rather than a major injury.
Having an insufficiency fracture is a strong warning sign that your bones may be weaker than they should be and need evaluation to prevent future breaks.
An insufficiency fracture happens when bone that has become weak — usually from osteoporosis or low bone density — breaks under everyday forces like standing, walking, or a minor stumble. The bone itself is the problem, not the force applied to it. This is different from a typical stress fracture in a young, healthy athlete, where repetitive normal-strength bone eventually gives out from overuse.
Common sites include the hip (femoral neck), pelvis, sacrum, and spine (vertebral compression fractures). These fractures can be tricky to spot because they sometimes come on gradually, with vague aching pain rather than a dramatic injury, and may not show clearly on a regular X-ray at first. Imaging like MRI or bone scan, and sometimes a DEXA scan to check bone density, help confirm what's going on.
Because an insufficiency fracture is often the first clear sign that bone has become fragile, it's treated seriously — it counts as a fragility fracture and raises the chance of future breaks. Your doctor will look at pain management, healing, mobility support, and whether osteoporosis treatment or further bone health evaluation is needed going forward.
A break from everyday activity can be the clearest sign your bones are weaker than expected.
