What it means

Lumbar spine BMD is a measurement of bone density in the lower spine, usually vertebrae L1 to L4, taken during a DEXA scan and used to help diagnose osteoporosis.

Why it matters for your bones

Your spine is one of the two key sites doctors check to see how strong your bones are and whether that's changing over time.

Lumbar spine BMD is the bone mineral density measurement taken from the lower spine (usually vertebrae L1 through L4) during a DEXA scan. It's reported as both a raw density number and a T-score, which compares your result to a healthy young adult. Along with hip measurements, it's one of the two main sites doctors use to diagnose osteopenia or osteoporosis and to track changes over time.

The spine tends to respond fairly strongly to things that load it along its length, such as squats or deadlifts done safely, and it's also a site studied closely in research on both exercise and osteoporosis medications. Reported spine BMD changes vary widely across approaches in the research literature, but these numbers come from different studies, populations, and risk levels, so they aren't directly comparable to each other or to what any one person should expect.

It's worth knowing that spine readings can sometimes look falsely 'improved' because of arthritis, compression fractures, or calcified tissue nearby, which is why your doctor may also look at hip results or order imaging to interpret a report accurately. Any decisions about starting, adjusting, or stopping osteoporosis treatment based on a spine BMD result belong to your doctor, who can weigh your full health picture.
