What it means

OPG (osteoprotegerin) is a protein made by bone-building cells that blocks a signal called RANKL, which helps slow down bone breakdown.

Why it matters for your bones

The balance between OPG and RANKL helps determine whether your bones are breaking down faster than they're rebuilding, especially around menopause.

OPG, short for osteoprotegerin, is a protein your bone-building cells (osteoblasts) make to help control how much bone breakdown happens. Think of it as a decoy: it grabs onto a signal called RANKL before RANKL can reach the receptor on cells that break down bone, called osteoclasts. Less RANKL getting through means less bone resorption.

The balance between RANKL and OPG is a big part of what decides whether your bones are gaining or losing ground at any given time. Estrogen plays a key role here, it boosts OPG and lowers RANKL, which is one reason bone loss speeds up after menopause when estrogen drops and the RANKL/OPG ratio shifts toward more breakdown.

This same pathway is also the target of a prescription medication, denosumab, which works like a man-made version of OPG by binding RANKL to reduce bone breakdown. Because stopping this medication can lead to a rebound in bone turnover, any decisions about starting, continuing, or stopping it should be made with your doctor.

✦ The aha

OPG blocks the RANKL signal, and this OPG-RANKL balance shapes whether bones gain or lose density, especially at menopause.
