Primary osteoporosis
Also called: age-related primary osteoporosis
What it means
Primary osteoporosis is bone thinning that develops mainly from aging and hormone changes, not from another disease or medicine.
It's the most likely reason your own bones may be thinning as you get older, especially after menopause.
Primary osteoporosis is the most common type of osteoporosis, and it happens on its own as people age, rather than being caused by another disease or medication. It's split into two overlapping patterns: the bone loss that comes with menopause, when falling estrogen speeds up bone breakdown, and the slower, steady bone loss that happens in both men and women simply from getting older.
Doctors usually diagnose it after ruling out 'secondary' causes, like thyroid problems, certain drugs, or digestive diseases that can also weaken bone. A bone density test (DEXA scan) showing a T-score of -2.5 or lower, often alongside a fragility fracture, is the standard way this is confirmed.
Because primary osteoporosis is so tied to aging and hormone changes, it affects a huge number of older adults, especially women after menopause. Understanding this distinction matters because it shapes how your doctor thinks about monitoring, lifestyle habits, and whether medication makes sense for you.
