What it means

Subclinical hyperthyroidism is when blood tests show low TSH but normal thyroid hormone levels, meaning your thyroid is slightly overactive without full symptoms.

Why it matters for your bones

Even mild, silent thyroid overactivity can speed up bone turnover and gradually weaken your bones over time.

Subclinical hyperthyroidism means your thyroid-stimulating hormone (TSH) is lower than normal, but your actual thyroid hormone levels (T3 and T4) are still in range, and you may not have obvious symptoms. It can happen on its own or from taking more thyroid hormone replacement (levothyroxine) than your body needs.

Thyroid hormone controls how fast your bones remodel—the constant cycle of old bone being removed and new bone being built. When thyroid hormone activity is too high, this cycle speeds up, but the bone-removal step outpaces the bone-building step. Over time, each cycle leaves you with slightly less bone than before, which can lower bone density and raise fracture risk.

Because this condition can be silent, doctors often find it through routine TSH blood tests. If you're on thyroid medication, your prescriber will periodically check your levels to make sure the dose fits your needs; decisions about adjusting any medication should always go through your doctor. Bone density testing (like a DEXA scan) may also be reasonable if you've had long-term low TSH.
