Trabeculae
Also called: bone trabeculae, trabecular struts
What it means
Trabeculae are the thin, interconnected struts of bone that form a spongy, lattice-like network inside bones such as the spine and hip.
When trabeculae thin out or disconnect with age or osteoporosis, your bones can lose strength even if density looks only mildly reduced.
Trabeculae are the fine, lattice-like struts of bone tissue that make up the spongy interior of many bones, like the inside of a sponge or honeycomb. You'll find dense networks of them inside the spine (vertebrae), hip, and ends of long bones. Alongside the dense outer shell called cortical bone, trabeculae give bone its strength while keeping it relatively light.
These struts aren't arranged randomly. They line up along the actual directions of stress and load the bone experiences day to day, a concept sometimes called Wolff's Law. In a healthy hip, for example, trabeculae form distinct patterns, some resisting compression and others resisting tension, like an internal engineering truss.
With osteoporosis, bone breakdown gradually outpaces bone building, and trabeculae become thinner, fewer in number, and less connected. This loss of architecture can weaken bone even beyond what a simple density number shows, which is part of why bone quality, not just bone mineral density, matters for fracture risk.
How sure is this?
Graded Established Science: Supported by broad scientific consensus and decades of research. Read each study with its grade in mind. One study is a piece of the picture, not the whole of it.
Thinning or disconnected trabeculae can weaken bones even when density looks only mildly reduced.
Where this leads
WordIf density alone can miss weakening trabeculae, how would you actually detect that? Trabecular Bone Score estimates how well-connected your bone's internal scaffolding is, beyond simple density.
