Vertebral endplate
Also called: endplate fracture
What it means
The vertebral endplate is the thin layer of bone and cartilage at the top and bottom of each spinal vertebra, connecting it to the disc above or below.
Weak or fractured endplates are often an early sign of bone loss in the spine and can lead to compression fractures and height loss.
The vertebral endplate is a thin layer of cartilage and bone that caps the top and bottom of each vertebral body, right where it meets the intervertebral disc. It acts like a gasket, helping nutrients move between the disc and the blood supply in the vertebra, while also spreading out the forces that pass through your spine when you stand, bend, or lift something.
Endplates can weaken with age, osteoporosis, or disc degeneration, making them more likely to crack or buckle under stress. An endplate fracture often happens alongside a vertebral compression fracture, since both involve the vertebral body losing its normal strength. These injuries can occur from a fall, lifting something heavy, or sometimes with no clear trauma at all if bone density is low.
Symptoms can range from none at all to sudden back pain, especially with bending or twisting. Diagnosis usually involves imaging such as X-ray or MRI, and doctors will often check bone density with a DEXA scan if an endplate or vertebral fracture is found, since it can signal underlying osteoporosis. Treatment and activity decisions should always be guided by your doctor, based on your specific situation.
Weak or fractured vertebral endplates are an early sign of spine bone loss and can lead to compression fractures.
Where this leads
WordSince a fractured endplate is an early warning sign, you might wonder what happens if the problem progresses. Here's what a full compression fracture looks like.
