Bone resorption markers
Also called: resorption biomarkers, osteoclast activity markers
What it means
Bone resorption markers are blood or urine tests, like CTX, that measure how actively osteoclasts are breaking down bone at a given moment.
Tracking resorption markers can help your doctor understand whether your bones are breaking down faster than they're rebuilding, which matters for assessing fracture risk.
Bone resorption markers are substances measured in blood or urine that show how much old bone is being broken down right now. The most common one is CTX, a small fragment of collagen that's released when osteoclasts (the cells that dissolve bone) do their work. Because they reflect real-time activity rather than a snapshot like a DEXA scan, these markers are often used alongside bone density tests.
One interesting feature is that resorption markers like CTX follow a daily rhythm: levels tend to run highest in the early morning and lowest in the afternoon. For this reason, blood draws for these tests are often timed consistently to make results comparable.
Doctors may use resorption markers to see how active bone breakdown is, to help understand the balance between bone loss and bone building, or to check how someone is responding to a treatment over time. Interpreting these results, and deciding what (if anything) to do about them, is best left to your doctor, since timing, lab methods, and individual factors all affect the numbers.
How sure is this?
Graded Strong Clinical Evidence: Supported by multiple well-designed clinical studies in humans. Read each study with its grade in mind. One study is a piece of the picture, not the whole of it.
Blood or urine tests like CTX reveal how fast your bones are being broken down right now.
Where this leads
WordYou just read that CTX is one resorption marker, so what exactly does this breakdown speedometer look like in practice?
