Bone turnover markers
Also called: BTMs, bone biomarkers, turnover markers blood test
What it means
Bone turnover markers (BTMs) are blood or urine tests that measure how fast bone is being broken down and rebuilt right now. The two main ones are CTX (breakdown) and P1NP (building).
They can show if your bone remodeling is speeding up, slowing down, or responding to treatment weeks or months before a DEXA scan would show any change.
Think of a DEXA scan like an odometer — it tells you how much bone you have right now. Bone turnover markers are more like a speedometer: they tell you which direction your bone remodeling is heading, and how fast, long before that shows up on a density scan. CTX (C-terminal telopeptide) is released when old collagen is broken down by osteoclasts, so it reflects resorption. P1NP is released when osteoblasts build new collagen, so it reflects formation.
These tests are most useful for tracking changes in yourself over time, not for comparing your numbers to someone else's, since normal ranges are wide and results vary a lot person to person. CTX in particular swings a lot across the day and drops after eating, so it needs to be drawn fasting in the morning for results to mean anything when compared to a past result. P1NP is more stable throughout the day.
Doctors commonly use these markers to check whether a bone medication is working as expected, to look at adherence, or to watch for a rebound in bone turnover after stopping certain treatments like denosumab. They are not used to diagnose osteoporosis on their own, and any decisions about starting, adjusting, or stopping treatment based on these results belong to your doctor.
How sure is this?
Graded Strong Clinical Evidence: Supported by multiple well-designed clinical studies in humans. Read each study with its grade in mind. One study is a piece of the picture, not the whole of it.
CTX and P1NP are blood tests that can show changing bone turnover months before a DEXA scan would.
Where this leads
WordYou just saw CTX named as the breakdown marker, so what does it actually measure and how is it tracked over time?
