Hypogonadism
Also called: low testosterone bone loss, low estrogen bone loss
What it means
Hypogonadism is when the body produces too little estrogen or testosterone, which speeds up bone loss because these hormones normally help keep bone breakdown and bone building in balance.
Low sex hormones accelerate bone loss, so recognizing and monitoring hypogonadism helps protect your skeleton before fractures happen.
Hypogonadism means your body makes lower-than-normal amounts of sex hormones—estrogen in women, testosterone in men. Because these hormones help keep bone-building and bone-breakdown in balance, low levels tip things toward faster bone loss. This happens naturally at menopause, but it can also be caused by certain medical conditions or treatments.
A common cause is treatment-induced: androgen deprivation therapy for prostate cancer and aromatase-inhibitor therapy for breast cancer deliberately lower sex hormones to fight cancer, but as a side effect they speed up bone loss—often faster than natural menopause. Other causes include GnRH agonists, long-term glucocorticoid use, chronic stress and high cortisol, and certain genetic or pituitary conditions.
If you're on one of these treatments, or have symptoms of low testosterone or early menopause, it's worth asking your doctor whether anyone is actively monitoring your bone density. Bone-protective steps—like baseline DEXA scans, adequate calcium and vitamin D, and weight-bearing exercise—are often recommended alongside these therapies. Never stop a prescribed hormone or cancer treatment on your own; decisions about managing hypogonadism always belong with your prescriber.
How sure is this?
Graded Strong Clinical Evidence: Supported by multiple well-designed clinical studies in humans. Read each study with its grade in mind. One study is a piece of the picture, not the whole of it.
Low estrogen or testosterone speeds up bone loss, so recognizing hypogonadism early helps protect your skeleton before fractures happen.

WordYou learned low sex hormones accelerate bone loss. Curious how low testosterone specifically weakens bone through its link to estrogen?
