Low testosterone
Also called: low T, testosterone deficiency
What it means
Low testosterone is when a man's testosterone levels fall below the normal range, which can speed up bone loss over time.
Low testosterone can weaken bones indirectly, since testosterone is converted into the estrogen your skeleton actually relies on most.
Low testosterone, sometimes called low T or testosterone deficiency, means a man's testosterone levels have dropped lower than what's considered normal. It's often connected to aging, but it can also happen earlier due to certain health conditions. For bones, the important twist is that testosterone itself isn't the main driver of bone strength in men — it's mostly about what testosterone becomes.
Testosterone gets converted into oestradiol (a form of estrogen) by an enzyme called aromatase, and oestradiol is actually the hormone doing most of the heavy lifting for male bone density. Men with very low oestrogen action, even with normal testosterone, tend to have weak bones. This means low testosterone matters for bones largely because less testosterone means less oestradiol gets made.
One tricky part of diagnosis: total testosterone numbers can be misleading, especially as men age, because a protein called sex-hormone-binding globulin rises and can bind up testosterone, making the free or usable amount lower than the total number suggests. Getting a fuller picture (morning blood draws, repeated testing, SHBG levels) gives more accurate answers. If low testosterone is suspected or being treated, bone health is a reasonable topic to bring up with your doctor, especially if any medication affects oestradiol levels.
How sure is this?
Graded Strong Clinical Evidence: Supported by multiple well-designed clinical studies in humans. Read each study with its grade in mind. One study is a piece of the picture, not the whole of it.
Low testosterone can weaken bones indirectly because it's converted into the estrogen your skeleton depends on most.
