What it means

The Wnt signaling pathway is a cell-communication system that signals osteoblasts to build new bone; when it's blocked, bone formation slows or stops.

Why it matters for your bones

This pathway controls whether your bone-building cells are switched on or off, directly affecting how much new bone you make.

The Wnt signaling pathway is a chemical messaging system that tells bone-building cells, called osteoblasts, to switch on and get to work. When Wnt signals reach the surface of an osteoblast, they bind to receptors called LRP5 and LRP6, which activates a protein called beta-catenin inside the cell. This process pushes young bone cells to mature and start laying down new bone matrix.

The pathway can also be shut off. Molecules like sclerostin and DKK1 block the Wnt signal from reaching its receptors, essentially putting the brakes on bone formation. Your body normally balances these 'on' and 'off' signals to keep bone remodeling steady, but disease and inactivity can tip that balance.

In multiple myeloma, cancer cells release DKK1 and sclerostin in excess, silencing the Wnt pathway and stopping osteoblasts from rebuilding bone, while other signals ramp up osteoclast activity to break bone down. This mismatch causes bone to be destroyed without being replaced, leading to the lytic lesions seen in this disease. Understanding this pathway is also why some osteoporosis medications are designed to block sclerostin, though any treatment decisions belong to your doctor.
