Phosphate test
Also called: serum phosphorus test, phosphate blood level
What it means
A phosphate test measures the amount of phosphorus in your blood. Phosphate works closely with calcium to build strong bones and teeth, and the test helps doctors check mineral balance and kidney or parathyroid function.
Phosphate levels that are too high or too low can be a sign of bone or kidney problems that affect how strong your bones stay.
A phosphate test, also called a serum phosphorus test, is a simple blood draw that measures how much phosphate is circulating in your bloodstream. Phosphate is a mineral that, along with calcium, forms hydroxyapatite, the hard crystal structure that gives bone its strength. Most of the body's phosphate is stored in bone, so blood levels give a window into overall mineral metabolism.
Doctors often order this test alongside a calcium blood test, parathyroid hormone test, and kidney function tests like eGFR, because phosphate, calcium, and vitamin D levels are all regulated together. Hormones such as parathyroid hormone and FGF23 help control how much phosphate the kidneys keep or release. An abnormal result can point toward conditions like chronic kidney disease, hyperparathyroidism, hypoparathyroidism, or vitamin D problems, all of which can influence bone health over time.
Low phosphate can sometimes contribute to softening of bone, as seen in conditions like osteomalacia or hypophosphatasia, while high phosphate is more often linked to kidney disease (CKD-MBD). If your results are outside the normal range, your doctor will look at the full picture, including other blood tests and your health history, to figure out the cause. Any follow-up or treatment decisions should be made with your prescriber, not based on this one number alone.
Phosphate levels that are off can point to bone or kidney problems affecting bone strength.
